The problem we kept seeing

In nursing home after nursing home, the pattern was the same. A CMS survey would conclude on a Friday. The exit conference would deliver a stack of deficiencies. And then, somewhere between the 2567 arriving and the 10-day PoC deadline, the real work would begin — in Word documents, email threads, and spreadsheets that no one could find.

Administrators and Directors of Nursing were spending their first two days after a survey just organising citations. Copying F-tag codes by hand. Building templates from scratch. Tracking corrective action deadlines in personal calendars.

For facilities under SFF designation or active CMP enforcement, the stakes were existential. And the tools available to them were the same ones every other industry stopped using a decade ago.

Why we built PoC360

We built PoC360 because the compliance burden in long-term care is real, the penalties are severe, and the administrative infrastructure — for most facilities — isn't built to handle it.

The CMS survey process is unforgiving. The 10-day deadline is fixed. The four required elements of a Plan of Correction are non-negotiable. And yet most facilities approach this process with no purpose-built tooling at all.

PoC360 starts from first principles: what does a nursing home team actually need in the 72 hours after a survey? A pre-populated workspace with citations already imported. Draft PoC language that gives the DON something to react to, not a blank page to fill. Task assignments that don't require a separate email chain. A deadline counter that's always visible.

What we believe

We believe that compliance software in long-term care has been designed for administrators, not for the people doing the work at 6 AM. It should be fast, focused, and do the rote work — leaving clinical judgment where it belongs: with the clinical team.

We believe that a facility on the SFF list isn't failing because their team doesn't care. They're failing because the administrative burden of CMS compliance compounds every other operational challenge they face. The right software doesn't solve every problem — but it removes the ones that shouldn't exist in the first place.

We believe that the best outcome for everyone — facilities, residents, and CMS — is a system that makes it easier to do the right thing, document it properly, and prove it when asked.

Where we are today

PoC360 is used by nursing homes across the United States — from single-facility independent operators to regional chains managing multiple locations. Our users include Directors of Nursing, Administrators, Quality Assurance leads, and the staff assigned to carry out corrective actions on the floor.

We're continuing to build — adding features that address the full CMS compliance lifecycle, not just the PoC window. Quality Measures, Staff Training, Compliance Programmes, QAPI — each built with the same principle: eliminate the administrative burden, surface what matters, and give clinical teams more time for care.
